Canada Life | Correction on Individual Life Insurance Application for Policy Change Form
Please note there is an error in Section 1 – General information, number 1.2 and 1.3 of the Individual life insurance application for policy change (form 17-8217) – in the financial question.
The following statement is incorrect:
Financial information: The following question is only required if the total insurance amount applied for exceeds $1,000,000 for that life.
Please ignore the $1,000,000 amount. This is incorrect. This wording should read as follows:
Financial information: The following question is required if an increase in insurance amount is requested for that life.
The financial Information question must be completed for increases in insurance amounts.
