BMO Insurance | New Insurance Life and CI Application Form

At BMO Insurance, we are committed to improving our New Business process. Our insurance application and client service forms have been re-designed to streamline the form completion process.

Revisions have been made to the BMO Insurance Application for Life Insurance and Critical Illness Insurance 126E and Policy Service Request Form 162E.  The attached email captures key improvements, transition rules and the launch logistics.  Advisors will receive the email on June 23, 2014.

As an MGA, you or your designated administrator registered with BMO Data Online will be able to order new applications starting June 17, 2014 for distribution to advisors.  To ensure a smooth transition, we will continue to accept old application forms until July 31, 2014.
