At BMO Insurance we are committed to improving our New Business process.  The following application forms have been updated to help streamline the form completion process.

Revisions have been made to the following BMO Insurance application forms:

1. (Face-to-Face) Application for Life Insurance and Critical Illness Insurance, 126E
2. (Face-to-Face) Application for Life Insurance insureNOW™ and insureNOW™ Plus, 633E
3. Non Face-to-Face Application for Life and Critical Illness Insurance, 431E

Changes to the (Face-to-Face) Application for Life Insurance and Critical Illness Insurance, 126E include:

• Application for Life Insurance and Critical Illness Insurance has been re-named Face-to-Face Application for Life Insurance and Critical Illness Insurance to further distinguish the two sales processes that BMO Insurance offers.  Please ensure you are using the correct application form for a face-to-face sale versus a non face-to-face sale.

• Section 4.2 – Authorization for Pre-Authorized Cheque (PAC)
  • For increased clarity, additional tick boxes have been added to the PAC Agreement set up instructions.
• Section 5 – Beneficiary Information
  • For increased clarity, an additional instruction has been added to define how Critical Illness proceeds from supplemental riders will be paid.
• Section 6 – Insurance History
  • For increased clarity, questions 2a. and 2b. have been re-written.
• Section 14 – Advisor Report
  • Question 3. The Advisor Certification has been updated in order to clarify that you have met with your client face-to-face.
• Section 16 – Application for Temporary Insurance
  • Question 2.a) has been updated.

Where applicable, the changes have been reflected in the (Face-to-face) Application for Life Insurance insureNOW™ and insureNOW™ Plus, 633E and the NON FACE-TO-FACE APPLICATION Non Face-to-Face Application for Life and Critical Illness Insurance, 431E.

Transition Rules

• The [updated] BMO Insurance Face-to-Face Application for Life Insurance and Critical Illness Insurance (126E), Application for Life Insurance insureNOW™ and insureNOW™ Plus (633E) and the Non Face-to-Face Application for Life and Critical Illness Insurance (431E) can be used immediately.
• All applications received in head office after December 9, 2015, must be submitted using the new application forms
• During the transition period, old application forms are still acceptable.
