Revised change forms effective July 1, 2014

On February 5, 2014, Canada signed an inter-government agreement with the United States to comply with FATCA, the Foreign Account Tax Compliance Act. The new legislation, called The Canada US Enhanced Tax Information Exchange Agreement, becomes effective July 1, 2014.

To make changes on existing life insurance policies, advisors must submit them on revised forms that conform to the new legislation:

• Application for Change
  (for Life Conversions) PC 2638
• Ownership Change
  (to change the owner on Life and Savings products) PC 7697

These forms are not available in print; advisors may print them from Advisor Portal as needed.
