La Capitale | New Applications T073 and T120

The Life and Critical Illness (T072) and Long Term Care (T119) applications have been updated. They will be available on April 1, 2014.

We ask that you please use the new versions exclusively. You can order them and you can also find them:

• in the La Capitale Sales Documentation section of our product illustration software, and
• on our website, in the Brokers section.

To help smooth the transition and allow everyone to make the necessary requests and supply their brokers, we will accept the old version until May 30, 2014.

Here are the main changes made to the different applications:

Life and Critical Illness (T073)

• Addition of investment accounts
• Addition of the 100% Pure Evolvement product
• Change to the insurance history in Children’s Critical Illness and Life Insurance riders
• Change to guidelines pertaining to the designation of minor irrevocable beneficiaries
• Addition of a question concerning bankruptcy
• Addition of an advisor’s declaration in the advisor’s report
• Addition of a confirmation number for ordering underwriting requirements

Long Term Care (T120)

• Removal of additional questionnaires (Section B)
• Addition of an advisor’s declaration in the advisor’s report
